This diary provides important information which will be helpful during your gastroesophageal reflux disease treatment and allows documentation of your symptoms along with their severity and frequency that will be self-assessed by you during the study. You can also record any side effects observed by you during the study treatment.
A diary helps to determine appropriate intervention and for tracking of your response to health measures interventions. Filling the diary on a regular basis will help your doctor to assess your health condition and allow him/her to provide the best possible care for treatment and change of medication if required.
Using the Diary, you will record each of the following:
1. Severity and frequency of heartburn
2. Severity and frequency of regurgitation
3. Any side effects observed during the study duration
4. Record of use of rescue medication
